Causes:
Trauma 90% (increased incidence in periorbital trauma)
Nontraumatic - Increased CSF pressure or primary nontraumatic CSF leaks
Sites:
Most common cribriform plate and ethmoid air cells
Frontal sinus
Sella turcica
Sphenoid sinus
Lateral lamella of cribriform plate
Posterior ethmoid roof near anterior and medial sphenoid wall
Natural history:
Most of CSF leaks resolve spontaneously
CSF fistulae > 7 days - increased risk of meningitis
Diagnosis:
Beta-2 transferrin assay is the test of choice (high sensitivity and specificity)
